HOURS OF OPERATION & LOCATIONS

We love seeing our patients and want to make sure we have plenty of time to spend with you. While you can stop by anytime for general medicine, we highly recommend making an appointment. For dental and pediatric services, appointments are required. Just make sure to register either on the day of your visit or beforehand, and have a payment method ready. We’re here to make your experience as smooth as possible!

ELIGIBILITY POLICY

We believe everyone deserves access to quality healthcare, regardless of their financial situation. That’s why we offer a Sliding Fee Scale for eligible families who don’t have insurance or qualify for other assistance. This program provides discounts on doctor visits and other services at PHC. If you’re interested in learning more about our eligibility policy and whether your family qualifies, please don’t hesitate to contact us. We’re here to help!

Sliding fee scale

Pasadena Health Center offers a sliding scale fee discount for patients who qualify. This reduction is based on the number of dependents residing at the patient’s residence, as well as total income earned in the household. If you have a problem making payment for services, discuss this with our staff before services are rendered.

 

To be considered eligible for the sliding fee discount you must present the following:

  • Valid Texas Drivers License or Texas ID (for Adults)
  • Valid ID= Birth Certificate (for each child)

In addition, the following must also be presented:

  • 2 paycheck stubs for each person in the same household that is working.
  • If you do not get paid by check, then a letter from an employer stating the amount of monthly gross earnings (or $ per hour and number of hours worked per week).
  • Proof of assistance from Texas Unemployment Commission (if receiving checks from the Commission.
  • Proof of any other financial assistance (child support, food stamps, etc.)
  • Letter of proof if a dependent in the same household is a full time college student.
  • Medicaid eligibility letter: this should be the same month the patient is being seen.
  • Bank Statement – last two (have to be current)
  • Income Tax – latest one filed
  • For eligibility renewal – need to present new current documents
