HOURS OF OPERATION & LOCATIONS
We love seeing our patients and want to make sure we have plenty of time to spend with you. While you can stop by anytime for general medicine, we highly recommend making an appointment. For dental and pediatric services, appointments are required. Just make sure to register either on the day of your visit or beforehand, and have a payment method ready. We’re here to make your experience as smooth as possible!

PAYMENT OPTIONS
At Legacy, we believe health care is a right, not a privilege.
We believe that comprehensive health care is a human right. That’s why Legacy’s services and programs are open to all who need us, regardless of the ability to pay, without judgment or exception.
Legacy accepts most health insurance plans, including HMO/PPOs, Medicare, and the following CHIP and Medicaid plans: Amerigroup, CHIP, Community Health Choice, Medicaid, Molina Health Care, Superior Health Care, Texas Children’s Health Plan, and United Healthcare. Eligibility Specialists are available at all Legacy locations to discuss sliding scale fees and potential programs to help reduce the cost of services. You may make an appointment with one of our Eligibility specialists by calling (832) 548 5000 or asking a front desk staff member. You can learn about all of our payment and assistance options below.
Health Insurance Plans Accepted
Legacy Community Health currently accepts the health insurance plans listed on our website.
In order to better serve you, changes are made to this list over time. Please address any questions about your plan with a Legacy team member when making your appointment.
Please remember to show us your insurance card when you arrive for your appointment. If you do not let us know about your coverage, you will be charged for past visits. For patients with a plan that needs a primary care provider (PCP) to be listed, either Legacy or a Legacy provider must be your PCP with your plan.
Medicare
Legacy Geriatricians provide a wide variety of services, including new patient consultations, annual wellness exams, and linking patients and caregivers to resources and support. If you’re about to become or have recently become eligible for Medicare, we encourage you to schedule your Medicare Initial Preventive Physical Examination (IPPE) at Legacy Community Health.

ELIGIBILITY POLICY
If You Are Currently Uninsured, Legacy Can Help You Access Health Care
Accessing health care can be frustrating, confusing, and even intimidating. If you are currently uninsured, Legacy’s Eligibility specialists can help you navigate the health care landscape and help you find the right combination of grants, government assistance, and sliding fee scale to make your health care more affordable. Eligibility specialists are available to anyone who walks through our doors and are available at all of our clinic locations.
Sliding fee scale
Patients who do not qualify for assistance programs can receive a medical visit for a lower rate based on income and family size. The sliding fee scale is determined through a meeting with a Legacy Eligibility specialist.
Please have your picture ID, insurance card, and form of payment ready. We accept cash, debit cards, credit cards, and money orders. Checks are not accepted. Legacy Community Health serves patients regardless of their ability to pay. Discounts for essential services are offered based on family size and income. Please ask a front desk team member for additional information.

You may make an appointment with an Eligibility specialist by asking at the front desk of any of our clinics or by calling (832) 548 5000. You will need to supply proof of your identity, income, and other information.
While our goal is to provide affordable care for all our patients, payment must be made at the time of service. Not having payment at the time of service may result in your appointment being rescheduled.
